Note: You must activate your account in Optimantra in order to view your treatment plans, lab orders/results, and other clinical information. You can also use your portal to book and reschedule appointments.
Fees
Initial Consult 60 minutes: $425
Follow-up 50 minutes: $375
Follow-up 25 minutes: $200
We will provide you with a superbill after your consult for submission to your insurance for out-of-network reimbursement.
